Health Insurance can offer benefits in three broad categories: (a) reimbursement of the cost of medical treatment/nursing care; (b) a periodic income upon disability or hospitalisation; or (c) a lump sum upon disability or a major illness/surgery. Medical Expense Insurance, also called Hospital & Surgical (H&S) Insurance, sits in the first category β it provides inpatient and some outpatient benefits, covering everything from daily room & board and surgical fees to complex procedures (heart bypass, organ/stem-cell transplant) and costly outpatient treatment (kidney dialysis, cancer chemotherapy/radiotherapy). In Singapore it is sold as a stand-alone policy. Examples include Medisave-approved Integrated Shield Plans (IPs), the MediShield Life Scheme, and Managed Healthcare Schemes.

Medical Expense Insurance, also called Hospital & Surgical (H&S) Insurance, sits in the first category β it provides inpatient and some outpatient benefits.
Cover ranges from everyday hospital charges (daily room & board, ICU, short-stay ward, surgical/anaesthetist/theatre fees, implants/prostheses) to complex procedures (heart bypass, major organ and stem-cell transplants, radiosurgery) and costly outpatient treatment (kidney dialysis, cancer drug treatment/services, radiotherapy). Some plans also extend to emergency medical evacuation and private nursing-home care.
In Singapore it is sold as a stand-alone policy, and most plans let the owner insure immediate family (spouse and children), sometimes with a family discount (e.g. 5%). Plans come in tiers by ward/hospital type β lower-tier plans suit 4-to-6-bedded public-hospital wards, higher-tier plans suit single/double or private rooms. Examples: Medisave-approved Integrated Shield Plans (IPs), the MediShield Life Scheme, and Managed Healthcare Schemes.
Medical Expense Insurance pays on a reimbursement basis β the insurer reimburses the policy owner the actual amount incurred (for "as charged" plans) or the maximum limit in the Schedule of Benefits, whichever is lower. The insured normally pays the bill first and then files a claim.
The owner can never claim more than the actual expenses incurred, nor more than the policy limits, so a full reimbursement is not guaranteed β the indemnity nature limits recovery to actual cost. Unlike life insurance (where multiple policies each pay their full sum assured on the same life), a person cannot stack Medical Expense policies to profit.
"As charged" vs sub-limited: many plans now cover key items "as charged" (the actual amount), which carry β but lifetime and annual limits still apply. Other plans impose per item (e.g. a table of surgical operations, or a room-&-board cap).
